Patient Forms
We see patients by referral only. Our physicians and staff are committed to providing the highest standards of care. Your initial visit will be with Dr. Gayles or Dr. Golovac and will entail a comprehensive evaluation, review of your medical records, review of any diagnostic tests that have been performed and discussion of an individualized diagnostic and therapeutic treatment plan.
Since pain management is considered a specialty, many insurance companies require a referral before they agree to cover the cost of services. It is the patient’s responsibility to obtain the referral from the primary care physician. Should one of your physicians recommend a procedure, we will assist in obtaining an authorization if necessary.
